Microsuction Earwax Removal – How does it work?

Microsuction is a procedure to remove earwax from the ear canal using a medically approved suction pump, a small suction wand and special attachments to loosen the earwax. The ear endoscope itself is a surgical instrument containing a fibre optic lens that has a light-emitting element around the circumference and a fixed lens in the centre. Traditionally, this procedure was only performed in ENT clinics in hospital outpatient departments, due to the size and weight of the original equipment. Thanks to new developments in portable suction pumps and visualisation equipment now available to suitably qualified clinicians, the procedure can now be performed by appropriately trained individuals in smaller clinics.

Microaspiration is a procedure to remove cerumen by suction. The instrument itself is usually a 2mm Zoellner tube, or a smaller 1.3mm thin end can be used when working closer to the eardrum, or in the rare case where children need assistance.

Why excess earwax needs to be removed

The ear canal forms earwax from the lining of the canal. Ears form wax to lubricate and protect the ear canal. The ear canal is the tube that connects the middle ear to the outside of the ear. Wax protects the ear from bacteria, infection and damage caused by water or trauma. Wax that forms in the canal naturally moves out of the ear and falls out. In some cases, the ear may produce too much earwax. If the wax is causing problems or preventing the doctor from seeing inside the ear, the excess wax can be removed.

Microsuction does not introduce water into the ear, but only removes earwax using a gentle suction method. The microaspiration surgeon uses a surgical microscope, called a “loupe”, and a bright light to see the inside of the ear canal in detail. This way, he can see exactly what he is doing in your ear. The suction cups are sterile, so the risk of infection is almost non-existent. For these reasons, the medical community considers microaspiration to be the safest method of earwax removal. The procedure can be quite noisy, but it is by no means uncomfortable. The UK Health and Safety Executive recommends that the best way to avoid permanent hearing damage is not to expose people to continuous noise levels of around 100db for more than 15 minutes without additional hearing protection. A smaller tube is used for the wax removal procedure in children over 12 years of age.
